Does Chocolate Cause Acne? The Real Research

The chocolate-acne myth is partly true — but not for the reason people think. Here's what the studies actually show.

The short answer

Partially true, but misnamed. Dark chocolate (high cocoa) has weak evidence of causing acne. Milk chocolate’s sugar + dairy drive acne more than cocoa itself. A 2016 double-blind trial showed 100% dark chocolate worsened acne in acne-prone subjects. Not proven for most people.

The chocolate guilt is legitimate — but not because of the cocoa.

What the research shows

The short answer

A 2016 JAMA Dermatology study (Caperton et al.) found that 100g of 100% pure cocoa daily worsened acne in acne-prone subjects within 48 hours. The mechanism is unclear — possibly cocoa’s effect on insulin signaling or inflammation. But this used massive doses far above typical consumption.

The reality:

  • Small amounts of dark chocolate: minimal effect for most
  • Large amounts of dark chocolate (100g+): may worsen acne in acne-prone
  • Milk chocolate: sugar + dairy drive acne more than cocoa
  • White chocolate: mostly sugar — drives acne

What actually drives diet-acne connection

The short answer

The diet-acne connection is primarily driven by: high-glycemic foods (sugar spikes trigger IGF-1, which increases sebum), skim milk (increases IGF-1 more than full-fat), and dairy in general. Chocolate is suspicious as guilty-by-association — it’s usually consumed with sugar + dairy.

The real food triggers:

  1. High-glycemic foods (sugar, refined carbs, white bread)
  2. Skim milk (counterintuitively worse than full-fat for acne)
  3. Whey protein (associated with cystic acne in some users)
  4. Dairy in general
