Okay, real talk. If you've been scrolling skincare TikTok for more than five minutes, you've heard someone say "just get tretinoin." They're not wrong.

Tretinoin — sometimes sold as Retin-A, Atralin, or Renova — is the only topical product the FDA has ever approved for both acne and photoaging. It's been studied since 1971. It works, we know why it works, and it costs less per month than one bougie latte.

What is tretinoin, actually?

The short answer
Tretinoin is all-trans retinoic acid — the active form of vitamin A your skin uses directly, unlike OTC retinol which your body has to convert in two steps.

Here's the TL;DR of vitamin-A skincare: your skin cells have receptors that listen for a specific molecule. That molecule is retinoic acid. Everything else in the "retinoid family" — retinol, retinyl palmitate, retinaldehyde, adapalene — has to be converted by your skin into retinoic acid before it does anything.

Tretinoin skips the conversion. It is the final molecule. That's why it works so much faster and stronger than OTC retinol.

The 20x myth is real. Tretinoin is roughly 20 times more potent than OTC retinol — it's not marketing, it's pharmacology.

How does tretinoin work on skin?

The short answer
Tretinoin speeds cell turnover from ~40 days to ~14-21 days, boosts collagen production in the dermis, and normalizes pigmentation — which smooths texture, fades spots, and clears acne.

Three things happen when tretinoin binds to your skin's retinoid receptors:

  1. Faster cell turnover. Fresh skin cells come to the surface faster, so damage, dullness, and dead-cell buildup get pushed out.
  2. Collagen synthesis. Fibroblasts in the dermis start producing more type I and type III collagen. Over 6-12 months, this plumps the skin from underneath.
  3. Melanin control. Hyperpigmentation fades because tretinoin tells melanocytes to chill.

How to get tretinoin (and what it costs)

The tretinoin purge — what to expect

The short answer
The tretinoin purge is a 4-8 week period of worsened breakouts at week 2-4, as tretinoin pushes existing clogged pores to the surface faster. It fades. Don't quit.

Here's the part nobody warns you about loudly enough: for the first month or so, your skin looks worse. You'll get tiny whiteheads in places you've never broken out. Your forehead will peel. Your nose will be flaky. This is called the purge, and it's real.

The timing usually goes: week 1 = fine, week 2 = "oh no," week 3-4 = full breakout, week 5-6 = starts clearing, week 8 = holy crap my skin.

Three things help:

  • The tretinoin sandwich method: moisturizer → tretinoin → moisturizer. Cuts irritation by ~50% without hurting efficacy.
  • Start at 0.025%, every third night, for the first 2 weeks.
  • Don't mix with AHAs, BHAs, vitamin C, or benzoyl peroxide for the first month. Pick your poisons later.

The tretinoin evening routine (step by step)

The short answer
Cleanse with a gentle cleanser, wait 20 minutes for skin to fully dry, apply a pea-sized amount of tretinoin, wait 20 minutes, seal with moisturizer. Only at night. Daytime: SPF 30+, always.
  1. Cleanse with a non-foaming, fragrance-free cleanser (CeraVe Hydrating works).

The biggest mistakes are starting too strong, applying to wet skin, layering too many actives, skipping SPF, quitting during the purge, and using more than a pea-sized amount.
  • Using too much. More does not work better. More just burns you.
  • Skipping moisturizer. You're not supposed to tough it out.
  • Skipping SPF. Tretinoin + UV = accelerated aging. You'll undo the entire benefit.
  • Using every night at the start. 2-3 times per week for two weeks, then ramp up.
  • Eye area confusion. Don't avoid it — just use less. The eye area benefits the most.
