The short answer

Pre-tretinoin prep: 2 weeks of gentle ceramide cleanser, niacinamide serum, and twice-daily ceramide moisturizer. No actives, no acids, no exfoliants. Result: stronger barrier, faster tretinoin tolerance, less severe purge.

The most-overlooked tretinoin tip: spend 2 weeks BEFORE starting building your barrier. Your purge will be 30-50% milder.

Why barrier prep matters

The short answer

Tretinoin disrupts the skin barrier as it accelerates cell turnover. A weakened barrier amplifies irritation. Two weeks of barrier-building before starting reduces TEWL (trans-epidermal water loss), making skin more resilient when tretinoin starts.

The barrier components you’re rebuilding:

  • Ceramides (lipid matrix)
  • Niacinamide (anti-inflammatory)
  • Hyaluronic acid (water retention)
  • Cholesterol (membrane integrity)

The 2-week prep routine

Morning (both weeks):

  1. Rinse with water (no cleanser)
  2. Niacinamide serum
  3. Ceramide moisturizer
  4. SPF 30+

Evening (both weeks):

  1. Gentle cleanser
  2. Niacinamide serum
  3. Ceramide moisturizer (slightly more than usual)

What’s NOT in the routine:

  • ❌ Retinoids
  • ❌ AHAs/BHAs
  • ❌ Vitamin C
  • ❌ Foaming cleansers
  • ❌ Anything labeled “exfoliating”

After prep: starting tretinoin

After 2 weeks of barrier prep, start tretinoin at the lowest strength (0.025%) using the sandwich method:

  1. Cleanse, wait 15-20 min
  2. Moisturize
  3. Wait 5 min
  4. Pea-sized tretinoin
  5. Wait 5-10 min
  6. Moisturize again

Apply 2x weekly for 2 weeks, then 3x weekly, then 4x, then 5x. By week 8 of starting tretinoin, you should be at 5 nights weekly with minimal irritation.

Frequently asked

Can I skip prep and just start tretinoin? +

Yes, but the purge will be more severe. Prep is optional but reduces irritation by 30-50%.

How long should prep last? +

Two weeks minimum. Some derms recommend 4 weeks for severely sensitive skin.

Should I keep using niacinamide after starting tretinoin? +

Yes — niacinamide pairs perfectly with tretinoin and continues to reduce irritation throughout the journey.
